CASE MANAGERS (HOMELESS SHELTERS)

Role Description This is a full-time, on-site role for Case Managers located in Amityville, NY. Case Managers will work directly with individuals experiencing homelessness to assess their needs, develop service plans, and connect them with housing, employment, and community resources. Responsibilities include providing ongoing support and advocacy, maintaining detailed case records, facilitating access to healthcare, and coordinating services with other organizations. The role requires collaboration with shelter staff to ensure the safety, well-being, and progress of each client while adhering to the agency’s values and policies. Qualifications Strong interpersonal, communication, and counseling skills to build meaningful client relationships and provide effective support.Experience with case management, service planning, and coordination of community-based resources.Proficiency in documentation, record-keeping, and navigating administrative systems.Problem-solving, organizational, and time management abilities to handle multiple cases efficiently.Familiarity with social services, housing programs, and resources for individuals experiencing homelessness.Bachelor’s degree in Social Work, Human Services, or a related field is preferred.Experience working with vulnerable populations and a strong commitment to ethical service delivery.Bilingual proficiency is a plus.Ability to work collaboratively in a team-oriented environment and maintain cultural sensitivity in diverse settings.
