Physical Therapist (PT) – Home Health / Outpatient | Day ShiftJob Summary:-We are seeking a compassionate and skilled Physical Therapist (PT) for a Home Health / Outpatient position based in Hidden Valley Lake (approximately 15 miles from Adventist Health Clear Lake). This is a great opportunity for a therapist who enjoys working independently while delivering high-quality patient care in a home setting.ScheduleMonday – Friday | 8:00 AM – 4:30 PMOptional weekend shifts available based on need and candidate preferenceKey ResponsibilitiesProvide physical therapy evaluations and treatments in a home health/outpatient settingDevelop and implement individualized care plans to improve patient mobility, strength, and functionAccurately complete and maintain documentation using Homecare Homebase (HCHB)Perform and document OASIS assessments in compliance with home health regulationsEducate patients and caregivers on therapeutic exercises, safety, and recovery plansRequired QualificationsLicensed Physical Therapist (PT) in the state of practiceMinimum 2 years of experience (Home Health preferred)Experience with HCHB (Homecare Homebase) – RequiredExperience with OASIS documentation – RequiredBLS/CPR Certification (BLS, HS-BLS, or RQI BLS)Call/Text - (551) 309-2468
