Job Description Utilizing a care team approach, coordinates patient care under the direction/supervision of the medical provider or registered nurse. The LPN is responsible for the delivery of patient care through the process of planning, implementation and evaluation of the patients served in the clinical practice. Works with Olean Medical Practice PLLC departments as well as outside healthcare entities to coordinate care for the patient as needed. Follows established guidelines and policies of the organization. Education And Experience Graduate of an approved program for practical nursing required. Currently licensed in the State of New York as a practical nurse or holds a work permit as required by the New York State Department of Education Division of Nursing. BLS Certification required.
