If you are interested in making a difference in your local community, this could be a great job for you!Community Paramedicine (CP) is an EMS model in which licensed providers deliver non-emergent, episodic care in out-of-hospital settings, such as patients’ homes, under physician direction. Services may include home visits, wellness checks, vital sign monitoring, specimen collection, vaccination, health education, and care transition support.The Community Health Assistant is a key member of the City of Bangor Public Health & Community Services and Fire Department teams. This position works closely with the Community Paramedicine Technician to coordinate referrals, scheduling, and workflows for the Community Paramedicine (CP) Program, ensuring timely, person-centered services for referred individuals.This position is grant-funded through the Community Paramedicine Serving Rural Mainers Project, supported by the Community Care Partnership of Maine. Funding is secured through November 2026; continued employment is contingent upon grant renewal or alternative funding.When operational capacity allows, this position may support Bangor Public Health Immunization and STI, programs within scope of practice. Duties may include point-of-care STI testing, specimen collection, vaccine administration, and participation in fixed and mobile clinics. These added activities are funded by the City of Bangor.
