OUTREACH bridges the gap between traditional outpatient rehabilitation and home health by providing individualized, patient-centered rehabilitation care in patients’ homes, senior residences, assisted living communities, and office settings.Due to continued growth, we are seeking compassionate, motivated, and clinically driven Speech-Language Pathologists who are passionate about working with geriatric populations and value flexibility, autonomy, collaboration, and meaningful patient care.Position SummaryThe Speech-Language Pathologist will evaluate and treat adult and geriatric patients within the community setting, focusing on communication, cognition, swallowing, speech, language, and overall functional independence.Clinicians work closely with patients, caregivers, and physicians to develop individualized plans of care that support safe, functional, and meaningful outcomes.Key ResponsibilitiesPerform comprehensive speech, language, cognitive, voice, and swallowing evaluationsDevelop and implement individualized treatment plansProvide speech, cognitive, communication, and dysphagia treatmentRecommend appropriate compensatory strategies and therapeutic interventionsEducate patients, caregivers, and family members on home programs and carryover strategiesCollaborate with Physical Therapists, Occupational Therapists, physicians, caregivers, and interdisciplinary rehabilitation team membersComplete timely and accurate electronic documentation in compliance with regulatory and company standardsQualificationsMaster’s Degree in Speech-Language Pathology from an accredited programActive New York State Speech-Language Pathology License requiredPreferred Clinical ExperienceDysphagia management and swallowing rehabilitationCognitive-communication rehabilitationNeurological rehabilitationVoice and speech disordersGeriatric rehabilitationHome-based rehabilitationMinimum 1 year of experience working with older adults preferredStrong clinical reasoning, communication, and interpersonal skillsAbility to work independently while remaining collaborative within a team environmentHome care, home health, outpatient geriatrics, or community-based rehabilitation experience preferredCompensation & BenefitsCompetitive unit-based compensation structure with treatment reimbursement up to $126 per session based on billed unitsFlexible scheduling opportunitiesComprehensive onboarding, mentorship, and clinical supportCollaborative and supportive team cultureEmployment TypePer Diem / PRNWhy Join OUTREACH?At OUTREACH, we believe rehabilitation is most impactful when care is delivered where patients are most comfortable — in their own environments.
