Primary Care Physician position with ChenMed in Jacksonville, FL.ResponsibilitiesIndependently assesses acute and non-acute clinical problems; performs and documents physical assessments and patient histories; analyzes trends in patient conditions; develops, documents, and implements a patient management plan.Assists in developing the nursing plan of care and provides patient, family, and significant other counseling and education.Plans patient care based on knowledge of the patient population and protocols; considers cultural background, level of understanding, personality, and support systems; serves as patient advocate.Performs patient management tasks including admissions, transfers, and discharges; orders and interprets laboratory and diagnostic studies; prescribes medications and treatments; refers patients for consultations when indicated.Documents progress notes and summaries; may perform invasive procedures upon documented competency; participates in rounds and conferences; communicates management strategies to the care team.Collaborates with multidisciplinary team to ensure patient management strategies meet care needs; recognizes emergencies and initiates life-saving procedures when necessary.Uses advanced communication skills to problem-solve complex situations and improve patient care processes and service.Collaborates with team to analyze and implement improvements in health care delivery; participates in external activities for professional growth; initiates SNF discharge arrangements and completes required paperwork.Works with physicians, nurses, PT, social workers, family, and caregivers to transition patients to appropriate levels of care; introduces self and role, facilitates patient/family conferences, and reviews goals of care and resource utilization.Supports discharge to appropriate level of care and coordinates with preferred providers for additional services; prescribes medications based on practice state guidelines.Other duties as assigned and modified by management.Knowledge, Skills and AbilitiesDemonstrated track record of achieving clinical performance metrics; strong critical thinking and problem-solving skills.Excellent communication and interpersonal skills; effective time management; empathetic, detail-oriented, and attentive to patient charts and follow-up.Proficiency in basic computer skills and Microsoft Office; ability to work in a fast-paced environment.Willingness to travel locally (this position may require 50-75% local travel).Education and Experience CriteriaMD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty required.Must obtain or already hold a current, active State Medical License for the state(s) of practice.Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty is preferred; Board Eligibility is required.Upon board certification, maintain certification through MOC, CME, and/or retaking exams as required.Current DEA number for schedule II-V controlled substances; Basic Life Support (BLS) certification within first 90 days of employment.We are transforming healthcare for seniors and offer competitive compensation, comprehensive benefits, and opportunities for career development and advancement.
