CTH Care Coordinator - Foster Care Community Training Home

Growing Home Southeast is looking for a CTH (Community Training Home) Care Coordinator This position will serve statewide. Remote Work opportunity. Candidate will need to reside in South Carolina. Growing Home SE CTH Care Coordinators supervise the consumer placement of young adults and families in CTH home setting to ensure the responsibility for case management and coordination of services for consumer.. Scope: CTH Care Coordinator is responsible in focusing on assessing those who meet criteria for DDSN services in order to address physical and mental health needs. Primary areas of focus for CTH Care Coordinators are facilitating communication between stakeholders, advocating on behalf of individuals that meet criteria for DDSN services, assessing and reviewing these individuals’ needs, and identifying appropriate interventions that promote a greater level of functioning.. CTH Care Coordinators are responsible for managing care services for individuals in CTH1 residential home setting. CTH Care Coordinators will work with individuals caregivers, case managers, and other providers to identify goals for improvement, which may focus on improved physical, mental, emotional, or vocational functioning. An essential feature of a CTH Care Coordinator job is to help the individuals to get the services they need in order to achieve their goals, as well as aid in procuring the services needed. Position responsibilities include, but are not limited to: Ensures regulatory compliance related to DDSN and DHHS regulations. These duties and responsibilities include monitoring, administration, documenting, staffing, scheduling, training and in-servicing of caregivers and/or individuals. Responsible for the supervisory functions of the CTH1 home. Ensures that all documentation required for the consumers Individual Support Plan have been completed, and monitors the safety and care of consumers. Ensures active treatment recommendations for clients in assigned caregivers homes are implemented. Monitors and/or implements behavior data sheets, ISP training programs, and GER (General Event Reports) to ensure all required documentation is entered into THERAP. Responsible for reporting significant events and suspected abuse immediately per SCDDSN policy and procedures, T-Logs, monitoring cash on hand for clients and MARS (Medication Administration Records. Monitoring and documenting progress toward goals, and planning transitions for discharges. Facilitate a monthly or quarterly support/training group for licensed caregivers. CTH Care Coordinator’s supervise the performance of caregivers and are responsible for maintaining quality care for each individual’s placement in accordance with Growing Home SE's Performance Improvement Plan. CTH Care Coordinator are members of an empowered clinical work team and may coordinate or assist with team-based budgeting, recruitment and hiring of team members, and continuous quality improvement. Assist, as determined by supervisor, in the recruitment, assessment, training, orienting, and licensing and/or re-licensing of contracted caregivers according to the established agency policy, accreditation standards and state licensing rules. Provide ongoing supervision to licensed caregivers and perform annual caregiver evaluations. Consult and collaborate with caregivers to include Identify problem areas with caregivers; develop a plan for addressing those problems; and assess placement progress to minimize the risk of placement disruption. Assist in enhancing the skill level and professional status of caregivers. Performs additional duties as assigned Maintain and manage a caseload of 10-12 consumers. Position Supervised: Independently contracted caregivers. Knowledge, Skill and Experience Required: Bachelor's Degree and one (1) year experience working with individuals with intellectual or developmental disabilities; or an equivalent combination of education and experience. CTH Care Coordinators should have knowledge of working with special needs adult population, behavior management, demonstrated ability to effectively work cross-culturally, and be able to approach interventions from an ecological framework. Skills in group and team facilitation, family assessment, and networking with community resources are essential. CTH Care Coordinators must demonstrate an ability to work independently, possess effective organizational and time management skills, and demonstrate professional oral and written communication skills. CTH Care Coordinators must be able to function under various amounts of stress and must have the self-knowledge to assure proper self-care and wellness. CTH Care Coordinators must possess a valid driver's license, have reliable transportation, be willing to work irregular hours. Preferred Qualifications Knowledge of federal and state regulations governing with persons with disabilities and special needs. Must exercise sound judgment, complying with all required regulations and procedures; must be a team leader and maintain effective working relationships. This position requires computer skills, excellent written and verbal communication skills. Must be physically able to work with consumers who may be physically aggressive or medically fragile.
