We are seeking compassionate and dependable RNs or LPNs to provide skilled nursing care and lead daily nursing activities in our skilled nursing community.Position Summary:The RN/LPN provides skilled nursing care to residents under the direction of the attending physician and in accordance with state nursing practice regulations. This role ensures compliance with facility policies while delivering compassionate, high-quality, resident-centered care.Essential Duties & Responsibilities:Provide nursing services within scope of practice, facility policies, and professional standardsMonitor residents for acute changes in condition and notify providers as appropriateManage and monitor chronic health conditions, reporting significant changesAssist with admissions, resident orientation, and discharge planningEnsure physician orders are in place for newly admitted residentsParticipate in MDS assessments (admission, quarterly, change of condition, etc.)Collaborate with the interdisciplinary team (IDT) to develop resident-centered care plansDeliver culturally competent, trauma-informed nursing careMonitor resident weight, nutrition, and hydration statusSupervise and provide guidance to CNAs under direction of the Director of NursingMaintain accurate and timely nursing documentation in EMR systemsCollaborate with hospice and palliative care providersPromote fall prevention and resident safety initiativesAdminister medications and monitor for adverse reactions or errorsAccurately transcribe and implement provider ordersAssess and manage resident painAssist with ADLs and resident repositioning as neededMonitor skin integrity and provide wound care as orderedCollect specimens for laboratory testingAdhere to infection prevention and control protocolsQualifications:Graduate of an approved RN or LPN programCurrent Illinois nursing license in good standingCPR certificationLong-term care experience preferredPhysical & Sensory Requirements:Ability to move throughout the workdayAbility to read, write, speak, and understand EnglishAbility to manage mental and emotional stressAbility to function independently with flexibility and professionalismAbility to lift, push, pull, or move at least 25 poundsAbility to assist with resident evacuation during emergenciesReasonable accommodations may be made to enable individuals with disabilities to perform essential functionsWhat We Offer:Competitive hourly rate: $33 - $41 per hourSign-On Bonus: $7,500 for Full-Time Registered Nurse (RN) or $6,000 for Full-Time Licensed Practical Nurse (LPN)Ask Us About Our Benefits:3% 401(k) MatchHealth, Dental, Vision, Disability & Life InsurancePaid Time Off (PTO)Tuition ReimbursementDaily Pay AvailableSupportive, team-focused work environmentHow to Apply:If you're passionate about providing exceptional care to others and are looking for a supportive and rewarding work environment, apply today!
