PRSC (Psychiatric Rehabilitation Services Coordinator)

All American Village Nursing and Rehabilitation Center - All American Village is seeking a compassionate, dependable Psych Rehab Services Coordinator (PRSC) to join our behavioral health team. The PRSC plays a key role in supporting residents with serious mental illness or psychosocial challenges by assisting with the delivery of psychosocial rehabilitation services. This role helps residents achieve greater independence and improve their quality of life through structured support, goal-setting, and participation in therapeutic programming.Key Responsibilities:Assist in delivering psychiatric rehabilitation services as outlined in individualized service plansSupport residents in developing life skills, social skills, coping mechanisms, and wellness routinesFacilitate or co-facilitate group and one-on-one psychoeducational and recreational activitiesProvide behavioral cues and redirection as needed in alignment with individual behavior plansDocument resident participation and progress in accordance with state and facility standardsCollaborate with the interdisciplinary team to monitor resident needs and communicate concernsMaintain a safe, supportive, and respectful environment that encourages participation and growthTransport or accompany residents to community outings or appointments when applicableAdhere to all HIPAA, safety, and infection control policiesQualifications:Required:High school diploma or GEDPrior experience working with individuals with mental illness, developmental disabilities, or in a behavioral health or long-term care settingStrong interpersonal and communication skillsPatience, empathy, and the ability to work in a structured therapeutic environmentAbility to document clearly and follow care plansMust pass background check and meet all facility and state employment requirementsPreferred:Associate's degree or coursework in psychology, social work, or a related fieldCompletion of a certified Psychosocial Rehabilitation Aide training (if required by your state)CPI, First Aid/CPR certificationWhat We Offer:Competitive pay based on experienceHealth, dental, and vision insurance optionsPaid time off and holidaysOpportunities for training and career growthSupportive team environment committed to quality behavioral health care
