Our reputation for excellence is built on great choices and empowered leadership, and fosters a team that is focused on evidence based practice, continual learning and exceptional quality of care.Job DetailsThe Physical Therapist is responsible for delivering direct care to patients and utilizing physical agents as prescribed by a referring physician.Responsibilities:Evaluates patient and creates treatment plan.Employee is able to provide verbal and written educational material to patients when appropriateRecognizes the need for involvement of other disciplinesCreates a recommended discharge planPerforms and records daily clinical notesParticipates in performance improvement initiativesCompletes department mandatory in-service/training/competency within specified time frameParticipates in education of staff and students within the medical centerRequirements:Bachelor of Science Degree in Physical Therapy or Master's degree or higher in Physical Therapy. Doctor of Physical Therapy (DPT) preferred.Licensed and registered PT in the State of New York.BLS required.Salary RangeUSD $44.41 - USD $55.55 /Hr.The pay for this position ranges from $39.00 - $50.00 per hour.This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate's qualifications, skills, competencies and experience.
