This full-time clinical position is 100% remote. The care team will work with NP daily to drive better outcomes for the entire panel. Ideal candidate is FNP licensed in one or more states in the Western U.S.What you'll do:Patient-Centered Quality and SafetyDemonstrate knowledge of etiologies, risk factors, pathophysiology, presentation, and epidemiology of medical conditions relevant to primary, acute, chronic, and transitional care patientsPerform history and physical assessments of patients that are accurate, complete, concise, relevant and appropriate for age, gender and clinical issueEvaluate and treat patients' medical needs with safe and high-quality careCounsel and educate patients and familiesDocument all patient care within an electronic health record in line with current policies and proceduresWork as a member of the care team providing care and coordination of our patients with internal and external colleagues ensuring the highest level of care is provided for all patients and at all timeProvide patient centered care that is compassionate, appropriate and effective for the promotion of health, prevention of illness and treatment of diseaseReferral to appropriate Specialists and patient care components as appropriatePossess an awareness and a working understanding of quality measurement in the management of chronic disease conditionsDocument patients that have expressed a desire to join care team panel, using approved EMR workflowsParticipate in/lead practice committees (e.g. Epic Optimization, Care Pathways, etc.)REQUIRED QUALIFICATIONS & ABILITIESActive Advance Practice Registered Nurse, Nurse Practitioner license in good standingCurrent National Board Certification, Family Practice5+ years of recent Primary Care experience as a Nurse Practitioner in family medicine or primary careEffective verbal, written, and electronic communication skillsOutstanding organizational skills and ability to multi-taskInitiative, problem solving ability, adaptability and flexibilityAbility to work remotely in a care team cultureIs proficient with information management and technologyObtain, and maintain in good standing, multi-state licensuresTake after-hours and/or overnight call to support patient careWORKING ENVIRONMENTVirtual Care Team members must meet minimum requirements for remote care delivery, including broadband connectivity, a quiet setting with a neutral background to conduct visits from, and the ability to uphold patient privacy per CVSH guidelinesWhile performing the duties of the job, the employee is regularly required to interact with patients remotely, sit, write, operate the computer and phone, speak intelligibly, and hear patient responsesSpecific vision abilities include the ability to view and read a computer screen and other electronic devicesEDUCATIONCompletion of master's degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role requiredAnticipated Weekly Hours40Time TypeFull timePay RangeThe typical pay range for this role is:$87,035.00 - $224,952.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.
