Come join our team!Check out the Great Place to Work link! https://www.greatplacetowork.com/certified-company/7044353Responsibilities:Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients within Houston area.Conduct comprehensive assessments of patients' social and emotional needsDevelop personalized care plans in collaboration with the interdisciplinary teamProvide counseling and support to patients and families facing end-of-life challengesAssist with coordination of care and access to community resourcesMaintain detailed and accurate documentation of patient interactions and progressUtilize Homecare Homebase EMR (knowledge of which is a plus)RequirementsLicensed as an LMSWKnowledge or experience utilizing Homecare Homebase EMR would be a plusMinimum of one year of hospice care experienceMinimum of one year of social work experienceCompassionate and empathetic approach to working with the elderlyStrong clinical and assessment skillsExcellent communication and interpersonal abilitiesAbility to work independently and manage time effectivelySalary Description
