Community Habilitation Counselor

Who We Serve Woodward Community Based Services was founded in 2006 in response to the growing need to serve children, adolescents, and their families in their home community. We provide a wide-range of programs and services to children, teens, and adults that are designed to help every individual and family we serve to reach their best and full potential. Our programs and services are available in our offices and in community settings, as well as in the homes of our clients. Group homes are also part of this level of care. Our professionals work in conjunction with local resources to develop a strength-based plan to support success for our youth and families. Home-Based HAB Hourly/SCL We are looking for someone to assist clients with working on their goals to increase independence in the community. This is a relationship-focused job where you will be working one-on-one with clients to build relationships and develop skills. You will work closely with your team as well as the client’s team to ensure the client is on track with services and to be sure their needs are being met. Job Requirements - 18 years or older - Maintain a valid driver’s license and be able to get a chauffer’s license through the DOT o Personal vehicle may be required to provide transportation - Complete service documentation within required time frame - Attend weekly team meetings - Punctual and reliable as you will be showing up to the client’s home for services · All other duties as assigned.
