At the Sharon at SouthPark our team member's number one responsibility is to deliver compassionate resident care every time!SummaryThe Licensed Practical Nurse is responsible for participating in assessing, implementing and documenting resident care as ordered by a physician and with the continuous availability of a registered nurse.ESSENTIAL DUTIES AND RESPONSIBLITIES Include The Following.Other duties may be assigned.Works with physician during outpatient clinic hours as assigned. Obtains vital signs, chief complaint, medication reconciliation, and ensures that orders are carried out accurately.Schedules appointments, follow up appointments. Calls patients for reminders.Provide education to residents and families regarding medications, disease processes and treatment recommendations.Documents and processes physician orders.Processes and follows up on referrals to outside specialists and therapies, including Vitality & Well-being.Performs assigned duties in the Wellness ClinicPrepares and administers medications and treatments as ordered by physician and documents appropriately.Performs phlebotomy and other lab services as ordered. Ensures orders are processed accurately.Participates in on-going resident assessment and evaluates resident's response to nursing actions under the guidance of a registered nurse.Documents changes in resident conditions and notifies physician, responsible party and supervising RN of changes.Keep physicians updated on resident's condition as needed.Ensures charges for nursing services and supplies are billed accurately.Participates in the development, implementation and review of service plans with the supervision of the Registered Nurse.Complies with company policies, procedures and related local, state and federal regulations.Remains current on nursing techniques and advances as applies to LPNs by attending in services, workshops, seminars, and continuing education opportunities.Takes "On Call" responsibility on a rotating basis as assigned.Adheres to the defined policy on the Bill of Rights for Nursing Home residents.Responds to emergencies that may occur on campus. Provide appropriate treatment within scope of practice. Completes appropriate documentation and other record-keeping duties as per company policies and relative local, state and federal regulations.Communicates with the residents, physician, family members, and other members of the health care team.Maintains a positive working relationship with residents, families, employees, other departments, management, and representatives of external groups.Performs other duties as assigned.QualificationsTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
