About the Facility - Prescott VAMC Community Based Outpatient Clinic - Cottonwood (CBOC) The Northern Arizona VA Healthcare System (NAVAHCS) is a Joint Commission-accredited, complexity level three facility serving over 27,000 Veterans across Northern Arizona. NAVAHCS provides a continuum of primary and secondary level medical, rehabilitative and long-term care to Veterans residing in northern and central Arizona. Acute medical care consists of primary and secondary inpatient medicine and ambulatory care, which includes general medicine, ambulatory procedures, and selected specialized medical clinics. The Emergency Department is open 24 hours/7 days a week. The VA Medical Center is located in Prescott, Arizona and Community Based Outpatient Clinics are located in Anthem, Cottonwood, Kingman, and Lake Havasu City, AZ. The Cottonwood Community Based Outpatient Clinic is a Group Practice with currently 7 physicians covering 6 specialty areas of medicine. Facility Address: 501 S Willard St, Cottonwood, AZ 86326 RESPONSIBILITIES (listed are included but not limited to) Services provided shall be performed in an ambulatory care setting at the Community-Based Outpatient Clinic (CBOC). Provider shall see patients for general medicine problems including performing the history and physical exam, making electronic progress notes, scheduling return visits, writing orders for lab and x-ray, prescribing medications, etc. Functioning as a primary care provider, the provider shall request as indicated, consultations from specialists and subspecialists in the WTVAHCS staff. Provider shall participate in quality management activities that pertain to his/her duties. In addition to the above clinical responsibilities, provider shall hold routine administrative duties. Shall participate in clinical conferences for staff and shall attend medical staff and service-level meetings. Services shall be performed in accordance with VA policies and procedures and the regulations of the medical staff bylaws of WTVAHCS. Provider shall enter all medical documentation in the Computerized Patient Records Systems (CPRS) and electronically sign all. Enter orders and prescriptions electronically using CPRS, as appropriate. REQUIREMENTS Must be Board Certified or Board Eligible by the American Board of Internal Medicine (ABIM) or American Board of Family Medicine (ABFM). Must have current license to practice medicine in any state, Territory, or Commonwealth of the United States of Columbia. Must have current certification in Basic Life Support at certified by the American Heart Association.
