This is an exciting opportunity to join a nationally recognized abdominal transplant program and to be a part of making a huge difference in patient's lives and developing expertise in transplant.About the RoleThe Transplant Nurse Coordinator provides patient care for transplant patients following established standards and practices. Functions to advance the clinical mission and goals of the MedStar Georgetown Transplant Institute through collaborative relationships within the Medical Center.We are currently seeking experienced nurses interested inPre Kidney TransplantPost Kidney TransplantPost Liver TransplantKey responsibilities includeCommunicates with potential transplant candidates and/or referring physicians.Facilitates the evaluation of both pre- and post-transplant patients by interfacing with referring physicians, Transplant hepatologists, Transplant nephrologists, Transplant surgeons, scheduling coordinators, consultants, insurance coordinators, and social workers. Primary responsibility for the planning implementation and evaluation of this process.Coordinates evaluation with the patient/family and appropriate members of the hospital, the transplant team, the patient's primary care physician and cardiologist and clinical consultants.Assess patients for placement on the transplant waiting list.Coordinates the complex management and education of patients' pre- and post-transplants.
