Team Select Home Care is looking for a Relationship Manager to join our Team Select family. The Relationship manager will work with both internal staff and patients and their families. They will also assist with payroll and scheduling of our field staff. The Relationship Manager will report directly to the Director of Operations.
Manage case load of 20-25 members.
Maintain a current awareness of the open positions for field staff in collaboration with Director of Nursing, Director of Operations, and scheduling department.
Maintain relationships with clients and employees and promote harmonious interpersonal relationships.
Maintain strict confidentiality on patient, agency, and employee matters.
Coordinate visit schedule for all field staff by staffing case for 60-day periods (or as ordered) with all applicable disciplines.
Ensure that all schedules are being followed and all documentation is submitted (missed visits, PRN visits and more).
Schedule only as directed by physician orders.
Attend weekly case conference at Director of Nursing's request.
Reconcile schedules with clinical itinerary records and perform computer system verification with the Administrative Assistant.
Help with payroll reconciliation/preparation as directed by Director of Operations.
Become a part of on-call rotation.
Demonstrate an ability to identify problems and initiative to reach quality decisions.
Willing to drive to visit clients for check-ins.

Requirements:
Graduate of an accredited high school or equivalent.
Have excellent clerical skills, i.e., writing, filing, typing.
