Registered Nurse - Home Health
Job DescriptionJob Summary:Provides comprehensive nursing care to patients in a home health setting, coordinating and directing services based on physicians' orders and agency policies. Responsible for ensuring high-quality patient care through accurate assessment, documentation, and supervision of home health aides and LPNs.Responsibilities:Coordinate and deliver individualized home nursing care in accordance with physicians' orders.Supervise, evaluate, and document the performance of Home Health Aides and Licensed Practical Nurses.Perform comprehensive assessments and develop tailored care plans for patients.Complete all documentation within 24 hours, ensuring compliance with agency standards.Travel to patient homes within a 75-mile radius, managing an average of 5-8 patients daily.Participate in periodic office meetings and maintain proficiency in EMR systems like HomeCare HomeBase (HCHB) and OASIS.Qualifications:Hold a valid RN license in the state of assignment.Possess at least 1 year of recent adult home health or hospice experience.Demonstrate proficiency in OASIS and HomeCare HomeBase within the last 6 months.Be able to travel up to 75 miles from the home clinic with reliable transportation.Have current CPR/BLS certification.Be willing to participate in on-call rotations and weekend shifts as required.Benefits & Growth:Enhance clinical skills through diverse in-home patient care experiences.Opportunity to work independently while collaborating with a supportive team.Gain flexibility with scheduling and travel within the service area.BenefitsDay 1 Medical, Dental, and Vision insuranceTravel reimbursement for assignmentsPaid sick leave and additional valuable benefitsAccess to our $1,000 Inspire a Friend Referral ProgramWhy TactStaff?Choosing TactStaff means partnering with a team that understands the unique demands and rewards of Home Health.
