If you want to feel honor and pride in the work that you do, while supporting a meaningful mission, come join our HopeWest Team!The Physical Therapist is responsible for:Performing comprehensive physical therapy assessments for new and existing participants.Developing individualized, realistic and goal oriented plan of care (POC) for each participant.Participating in Interdisciplinary Team (IDT) meetings daily, and plan-of-care meetings as assigned.Providing appropriate physical therapy interventions according to the POC.Educating the participants, family members, and/or caregivers about exercises and special techniques.What is PACE?PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.You Bring:Graduate from a CAPTE accredited Physical Therapy program.Currently licensed as a PT by the state of Colorado and maintained throughout employment.1 year of PT experience required; 2 to 3 years of home health or other home-based PT experience preferred.Valid Colorado driver's license, unlimited access to reliable transportation, and adequate automobile insurance is maintained throughout employment.Able to work in a variety of environments.Adequate computer literacy in MS Word, Excel, Access, OutlookCurrent CPR certification is maintained throughout employment.Excellent interpersonal and communication skills.Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.Effectively work with and build relationships with others.We Offer:Competitive pay structure starting at $43.38-$48.00 per hour, depending on experience.UMR Health, Delta Dental, and VSP Vision insurance effective the month following start date.Immediately start accruing PTO and medical leave.Colorado Family and Medical Leave Insurance (FAMLI) program.15% discount at Spoons Bistro & Bakery, and Heirlooms locations.Up to 3% match on 403B retirement.Mileage reimbursement for work related driving at 65 cents per mile.Employer paid life insurance and Employee Assistance Program.Tuition reimbursement programTogether, through creativity, volunteerism, and philanthropy we change the experiences of aging, illness, and grief - one family at a time.HopeWest honors our service members and encourages veterans to apply.
