Grace Reliant's Residential team is growing, and we're looking for a Community RN who is passionate about supporting individuals with intellectual and developmental disabilities. Our mission is to help people live more complete and independent lives.The Community RN completes monthly health assessments, oversees medications and documentation, and stays closely connected with our Residential team. You'll also guide and train staff through delegation, supervision, and classes like Level I Medication Aide and CPR/First Aid. This role is key to maintaining high-quality, consistent care for every consumer we serve.Duties and Responsibilities:Monitor the health and safety of the individuals receiving residential services.Completing a monthly face-to-face assessment of individual consumers and record findings.Review the individual consumer record monthly to include physician orders and the medication administration record.Maintaining communication with the DMH team including the reporting and documentation of all changes in the individual's health status, needs, and identified deficiencies in the standard of care provided to consumers.Provide appropriate delegation and supervision to Level I Medication Aide or Certified nursing assistant who performs such duties as medication administration and other nursing tasks when applicable, and document those activities.The Community RN will be responsible for:Identifying needs for staff supervision/ delegations or specialized instruction and any follow-up or plan of action that may result.Oversight of all functions of medications administration record and staff documentation.Provide quarterly education trainings to the staff to improve the level of care we provide.Ensure that specialized instruction regarding identified task is provided and documented on the delegation form.Ensure periodic oversight/supervision that the identified staff can perform the specific task as delegated.Account for their time dedicated to the functions of the Community RN. This is accomplished by the completion of the monthly service log, health care report, and communication notes each month.Provide Level I Medication Aide and CPR/First Aid classes monthly to ensure that staff are properly trained and hold the correct credentials to be able to provide care to our consumers.HRST. Act as the designated rater. This includes completing HRST monthly assessment.Other duties as assigned by supervisor.Qualifications include:Must be a graduate from an accredited School of Nursing.Must be licensed in Missouri as a Registered Nurse.Experience in community nursing, Dept of Mental Health and/or nursing case management.Should be skillful in organization and in the principles of time management and management process.Be compassionate, team-oriented, and competent in computers.Must possess a valid driver license and acceptable driving record.Must have reliable transportation and vehicle insurance.Demonstrate the ability to communicate effectively both in writing and verbally.Benefits:As a full-time team member at Grace Reliant, you'll enjoy:Comprehensive Benefits: Health care, dental, vision, FSA, etc.Flexible Work Schedule: Options to accommodate various personal and professional needs.
