Job Summary:We're seeking a motivated and experienced Assistant Care Coordinator to join our team. This full-time role requires a strong presence in the field, balancing administrative precision with hands-on support. As an ideal candidate, you'll have a minimum of 2+ years of experience in agency homecare or facility care, good availability including some weekends, and reliable transportation. Responsibilities:Field Support & Mentorship:Spend significant time in the field conducting caregiver introductions, performing on-site spot checks, and providing hands-on support to ensure care plans are being followed correctly.Shift Coverage:Provide direct client care and shift coverage as needed to ensure continuity of service during caregiver transitions or emergency call-outs.Client Onboarding Assistance:Assist the leadership team with the transition from lead to active client, ensuring all "Post-Orientation" protocols are met.Quality Assurance:Monitor caregiver performance and client satisfaction, acting as a bridge between the front-line staff and the office leadership.Administrative Coordination:Utilize Axis Care and Google Workspace to update care notes, track compliance, and assist with the seamless flow of documentation.Team Collaboration:Work closely with the Care Coordinator and Director of Nursing to identify staffing needs or clinical changes that require immediate attention.Requirements:Experience:Must have an active RI CNA license with previous experience in a homecare or healthcare setting.
