By promoting recovery and independence, we help patients achieve optimal health and comfort in a familiar and supportive environment.Role DescriptionThis is a full-time (from 10am-16pm), on-site role located in the Glendale for an Intake Coordinator. The Intake Coordinator is responsible for managing the intake process for new patients, ensuring accurate and timely documentation, verifying insurance coverage, and serving as the primary point of contact for patients, families, and referring providers. Day-to-day tasks include communication with healthcare professionals, maintaining patient records, coordinating case management efforts, and providing exceptional customer service to ensure a smooth admission experience.QualificationsPrevious experience as a home health intake coordinator or similar role is requiredStrong understanding and knowledge of home healthcare process, policies and proceduresProficiency with electronic medical records (Healthcare Synergy & WellSky/Kinnser), as well as Microsoft Office and Adobe Acrobat applicationsExperience in Case Management, including coordination of care plans and continuity of servicesStrong Communication and Customer Service skills, with the ability to engage effectively with patients, families, and healthcare professionalsAbility to work independentlyHigh school diploma or equivalent; additional certifications or a degree in a related field is a plus
