The individual holding this position is required to support and demonstrate their commitment to Longevity Health’s purpose: To optimize the quality of life of people in long-term care by focusing on their individualized health needs.The APP CDI Market Support Specialist is a clinically experienced Advanced Practice Provider (NP or PA) who supports Clinical Documentation Integrity (CDI) efforts across an assigned territory, with a strong focus on Skilled Nursing Facilities (SNFs) and Institutional Special Needs Plans (ISNPs). This role partners closely with APPs, physicians, CDI, Quality, and Revenue Cycle teams to improve documentation accuracy, risk adjustment, quality outcomes, and compliance with CMS requirements.This is a field-based role requiring up to 25% travel, including occasional overnight stays.Key ResponsibilitiesSupport and guide APPs in SNF and ISNP environments to improve clinical documentation accuracy and completenessProvide education on CMS guidelines, risk adjustment (HCCs), quality measures, and documentation best practicesCollaborate with CDI, Coding, Quality, and ISNP leadership teamsIdentify documentation gaps and trends impacting reimbursement and quality outcomesPromote consistent documentation standards across assigned marketsServe as a trusted clinical and documentation resource for providersRequirementsRequired QualificationsActive Nurse Practitioner (NP) or Physician Assistant (PA) licenseMinimum 3 years of experience in a Skilled Nursing Facility (SNF) setting as a Clinical Provider Minimum 2 years of direct experience working within an Institutional Special Needs Plan (ISNP)Strong knowledge of Medicare Advantage, ISNP populations, and CMS documentation requirementsExperience working with clinical documentation, risk adjustment, and quality measuresStrong communication and provider education skillsPreferred QualificationsCDI certification (CCDS, CDI-P, or equivalent)Experience supporting documentation improvement initiatives across multiple sites or marketsPrior provider education or clinical leadership experienceWork Location & TravelPrimary Location: Remote / Field-basedTravel Requirement: Willingness to travel up to 25% within an assigned territory, including occasional overnight staysSummaryCompany OverviewLongevity Health is a national provider of Institutional Special Needs Plans (I-SNP), created to meet the complex healthcare needs of nursing home residents.
