Case Manager - Healthcare for Homeless Veterans Program (HCHV)

Position Summary: Responsible for the full client care cycle including: assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients health and human services needs. Position Description: Develop and/or implement individualized client service plans, monitor progress utilizing supportive methods, and determine updates needed in collaboration with the client and team during weekly case conferencing. Work with offsite support, coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered according to plans developed in conjunction with our partners at the VA. Deliver on-going services to clients which may include: crisis intervention, advocacy, transportation assistance, budgeting, housing assistance, and referral and linkage to community resources as needed. Assist in helping clients achieve wellness and autonomy through supportive counseling and facilitating multiple care aspects (care coordination, information sharing, etc.). Maintain caseload and productivity by maintaining regular contact with clients to monitor and assess progress. Document all clinical activities to meet specific program, funder and credentialing entity criteria timely and with accuracy. Ensure compliance with applicable standards, agency policies, and contractual requirements. Adhere to professional standards as outlined by protocols, rules and regulations. May require transportation of clients in vehicle. Other duties as assigned. Required Knowledge, Skills and Abilities: Ability to interact with others in an ethical, culturally sensitive, and respectful manner. Be adaptive, flexible, and responsive to the needs of the client and program. Demonstrate professional behavior and strong clinical boundaries. Ability to teach clinical therapy principles. Excellent organizational and time management skills. Effective verbal and written communication skills. Advanced problem-solving skills and ability to multi-task while prioritizing tasks. Remain current in general knowledge of psychotropic medications, side effects, and DSM/IC diagnoses. Ability to use electronic service documentation systems effectively. Job Requirements: Bachelors Degree in Behavioral Science or related field, or equivalent work experience as allowed/required by regulatory bodies 2 5 years clinical counseling experience preferred Essential Functions/Physical Demands: Positional: Driving in accordance with job duties assigned.
