Seeking a Family Nurse Practitioner who will work as part of a multidisciplinary team providing coordinated physical and behavioral health treatment;Salary range $115k - $125K depending upon experience;M-F 35 hour work week;-Will interview consumers to obtain history, perform physical examinations, order labs and other tests, and prescribe medications and treatments.-Perform physical health assessments, defined as the physical evaluation of an individual, including an age and gender appropriate history, examination, and the ordering of laboratory/diagnostic procedures as appropriate.Assessment of mental status through mental status evaluation, medical, and social history takenImplement and provide continuity of physical health care to patients, work with patient to insure understanding of and compliance with established protocols, and coordination with health care providers.Ensure a high level of quality of care is maintained for the patient through the use of resources in the health care environment and through consultation with other health care professionals.Provide referrals to specialists when required.Prescribe and refill medications as needed.Create treatment plans.Prepare and update patient records in EHR.Required: experience in community-based primary care services, experience with behavioral health (serious mental illness, substance use disorders) preferred, must be computer literate with ability to enter information in the EHR, must be able to read, write and speak English.FNP?s must be certified and registered as a nurse practitioner by the State of New York and have 3+ years experience as a FNP.
