The APP will provide comprehensive, high-quality, evidence-based care to adult abdominal transplant patients across the pre- and post-transplant continuum within Duke Health ambulatory practice settings.This role functions within a highly collaborative, interprofessional transplant care model and practices to the full extent of licensure and training in alignment with Duke Advanced Practice standards. The APP supports the Division of Abdominal Transplant Surgery by enhancing access, continuity, quality, and outcomes of care in a nationally recognized academic transplant program.Clinical ResponsibilitiesObtain comprehensive medical histories, perform focused and complete physical examinations, and synthesize clinical data to guide careReview, interpret, and act upon diagnostic studies, including laboratory results and imagingDevelop accurate and comprehensive differential diagnoses and evidence-based treatment plansPrescribe, manage, and adjust immunosuppressive and transplant-related medications in accordance with departmental guidelines and credentialed privilegesProvide longitudinal management of pre- and post-transplant patients, revising plans of care as clinical conditions evolveManage post-transplant issues including rejection surveillance, infectious evaluations, and post-operative complicationsPerform medical procedures within the scope of practice, credentialing, and departmental standardsMaintain timely, thorough, and concise documentation to support care delivery, continuity, quality metrics, and compliant billingPrioritize patient flow and clinic efficiency while maintaining productivity expectationsParticipate in quality, safety, and clinical improvement initiatives specific to transplant careKnowledge, Skills, and AbilitiesDemonstrated clinical competence in the management of complex patients in a specialty outpatient settingStrong analytical, critical-thinking, and clinical decision-making skillsAbility to practice both independently and collaboratively within a multidisciplinary transplant teamEffective written and verbal communication skills with diverse patient populations and clinical stakeholdersStrong interpersonal skills that support patient care, teamwork, and clinic operationsDemonstrated proficiency in medical record documentation to substantiate clinical care and accurate billingEducation & Interprofessional CollaborationProvide clear, compassionate education to patients and caregivers regarding transplant conditions, medications, monitoring, and follow-upCollaborate with physicians, nurses, pharmacists, coordinators, and allied health professionals to ensure patient- and family-centered careAssist in the development of educational resources to support learners and clinical staffActively engage in continuing education and self-assessment to support professional growth and clinical excellenceProfessional Practice ExpectationsInteract with patients, families, and team members in a respectful, compassionate, and timely mannerCommunicate effectively across disciplines to ensure safe, coordinated, and high-quality careDemonstrate accountability for clinical decisions, outcomes, and professional responsibilitiesShare responsibility for team workload and support a positive, collaborative work environmentMinimum QualificationsEducationNurse Practitioner: Graduation from an accredited Nurse Practitioner programPhysician Assistant: Graduation from an accredited Physician Assistant programLicensure & CertificationNurse Practitioner:Active or compact RN licensure in North CarolinaNational NP certification requiredPhysician Assistant:Current North Carolina PA licensureNCCPA certificationBoth: Must demonstrate and maintain credentialing and licensing requirements of the DHIP and Duke Health.
