Shift Day Shift, Various Hours Scheduled Weekly Hours 24 POSITION PURPOSE: The Dementia Navigator will collaborate with the Primary Care and Neurology practitioner health teams and assist with coordination of all aspects of assisting patients with a dementia diagnosis in receiving assistance in finding support services and resources to aid in their care. Including but not limited to referral to services such as PT/OT/Speech, Nutrition Center, Ways to Wellness counseling, Behavioral Health Unit, and locating other resources such as CGH Health Foundation, Alzheimer's Association, Whiteside County Senior Center, Lifescape Community Services, Northwestern Illinois Area Agency on Aging, NICIL, home nursing, respite care, assisted living, long-term care, hospice, etc. In addition, this position provides dementia resources for the patient, family/caregiver, staff and community. ESSENTIAL FUNCTIONS: Serves as a liaison between the providers and patients/families in their dementia journey to identify, facilitate and secure access to needed healthcare, social services benefits, and community resources Reviews patients/families need for resources/assistance and works with them to care plan Informs patients/families on dementia process Communicates regularly with patients/families to support care plan goals and provide further guidance/assistance Oversees dementia care assistance fund Links caregivers with area support groups Attends area meetings/events such as Senior Fair to represent the hospital Maintains metrics on patients/families served Develops comprehensive listing of local, regional, and national resources/services/assistance funds Provides dementia information to staff and community SPECIALIZED KNOW-HOW & REQUIREMENTSAssociate Degree in relevant field of study or 5 years work experience.Familiar with MS Office and Electronic Medical Records systemsKnowledgeable about the breadth and accessibility of community dementia resourcesAbility to communicate openly and empathetically with patients, families, and medical providersExcellent organizational, interpersonal, oral, and written communication skills and ability to used critical thinking skills to problem solveAbility to occasionally work eveningsMust have current driver's license Compensation Range $16.00 - $24.00 Available Benefits for Benefit Eligible Positions Health and Dental Insurance IMRF (Illinois Municipal Retirement Fund) Retirement Fund (457 and 401A Plan) Medical and Dependent Care Flexible Spending Life Insurance Short and Long Term Disability Optional Insurances Paid Time Off Tuition Reimbursement and Loan Forgiveness Gift Shop Discount CGH Day Care Discount CGH Dietary Discount Employee Assistance Program CGH Pharmacy Discount CGH Vision Center Discount Thank you for your interest in a career at CGH Medical Center.
