Full-Time Physical Therapist (PT) Champion City Nursing and Rehabilitation Center – Pittsburgh, PA Full-Time | Flexible Schedule | Monday–Friday Preferred Champion City Nursing and Rehabilitation Center is seeking a dedicated and compassionate Physical Therapist to join our full-time therapy team. Position Summary The Physical Therapist is responsible for evaluating residents, developing individualized treatment plans, and providing skilled physical therapy services to improve strength, balance, mobility, and functional independence. The PT works closely with the interdisciplinary team to ensure optimal clinical outcomes and high-quality patient care. Responsibilities Perform comprehensive physical therapy evaluations and develop individualized plans of care Provide skilled interventions including gait training, balance training, strengthening, transfer training, neuromuscular re-education, and functional mobility training Assess and recommend appropriate assistive devices and mobility equipment Complete timely, accurate documentation in compliance with Medicare, Medicaid, and facility guidelines Collaborate with nursing, physicians, therapy team members, and interdisciplinary staff Participate in care plan meetings, discharge planning, and family education Screen residents for therapy needs and make appropriate recommendations Maintain productivity expectations while delivering quality, patient-centered care Ensure compliance with all federal, state, and facility regulations Qualifications Graduate of an accredited Physical Therapy program Current Pennsylvania Physical Therapist license (required) Skilled nursing facility experience preferred, but not required Strong clinical reasoning and documentation skills Excellent communication and teamwork abilities Ability to work independently and efficiently Benefits Competitive salary Medical, Dental, and Vision insurance Paid Time Off (PTO) 401(k) retirement plan Flexible scheduling Supportive leadership and therapy team Opportunities for clinical growth and advancement Why Join Champion City Nursing and Rehabilitation Center?
