Registered Nurse - Hospice - SLC County

Active Home Health, Hospice & Personal Care is looking for a Full Time Hospice RN to care for our patients in Salt Lake County. We are looking for someone who is caring, professional and self-motivated to join our team. We are a fun-loving, compassionate group that thrives off the gratification of caring for others. Starting salary commensurate with experience.Job Requirements:Visit and assess patients to complete an initial comprehensive Assessment and ongoing Assessments through end of care.Chart all clinical documentation in our EHR softwareCreate and manage patient plan of careEducate and give instruction to patient, family, and caregivers as appropriate per patient care plan.Case manage the patient episode or benefit period, ensuring that all cares are performed and documentedProvides medication education and management per physician orderWorks hand-in-hand with the IDG to ensure patient goals are met and appropriate outcomes are achievedShares on-call dutiesMaintains strong communication with patient, family, caregivers, and the interdisciplinary teamsJob Qualifications:Registered nurse with current license to practice professional nursing in the state of UtahPolite and compassionateExcellent observation, problem solving, verbal and written communicationsMaintains a current CPR certification1 year Home Health or Hospice experience preferredMust be a licensed driver with an automobile that is insured, registered and in good working orderGood communication and smartphone skillsAbility to pass a background checkBenefits:starting salary commensurate with experienceMedical, Dental and Vision insuranceLife insurancePaid vacationMileage reimbursementAwesome team environment and great co-workers401k
