The RN Case Manager will be responsible for coordinating patient care, facilitating safe and timely discharges, and ensuring appropriate utilization of healthcare resources while collaborating with interdisciplinary teams.Key ResponsibilitiesManage a caseload of inpatient patients across acute care units (Med-Surg, ICU, Telemetry, ED, etc.)Perform comprehensive patient assessments to identify clinical, psychosocial, and discharge planning needsCoordinate safe and timely discharge planning, including placement to SNF, rehab, LTAC, or home careCollaborate with physicians, nurses, social workers, and ancillary departments to ensure continuity of careConduct utilization review (UR) and apply InterQual / MCG criteria to determine medical necessity and level of careManage payer communication, including authorizations, concurrent reviews, and denialsFacilitate care transitions and coordinate services such as DME, home health, infusion, and hospiceEducate patients and families regarding treatment plans, post-discharge care, and available resourcesMaintain accurate and timely documentation in EMR systems (e.g., EPIC, Cerner)Ensure compliance with hospital policies, regulatory standards, and CMS guidelinesRequired QualificationsActive CA Registered Nurse license3+ years of RN Case Management experience in an acute care / inpatient hospital settingStrong experience with discharge planning and care coordinationKnowledge of InterQual and/or MCG guidelinesExperience with utilization review and payer communicationFamiliarity with EMR systems (EPIC, Cerner, or equivalent)Excellent communication and interdisciplinary collaboration skillsRequired Qualifications:Active CA Registered Nurse license3+ years of RN Case Management experience in an acute care / inpatient hospital settingAbout Prodware Solutions LLCProdware Solutions LLCis a healthcare staffing agency connecting nurses and allied health professionalswith trusted healthcare organizations nationwide.
