Share these schedules with the HCS Coordinator and HCS Rehab Technicians.Coordinate and schedule medical appointments for consumers within the program.Train, orient, and mentor staff to ensure high standards of service delivery.Address and resolve service-related issues with consumers, family members, and guardians in a timely and appropriate manner, ensuring follow-up as needed.Prepare and complete data reports for use in Periodic Reviews, predicted outcomes, and Global Outcomes analysis.Assist with intake paperwork and annual updates as requested by the clinical team.Support the implementation of safety protocols and ensure all required documentation is completed.Help with day-to-day scheduling and problem-solving within the program.Be available for after-hours issues, including participation in the on-call rotation.Oversee and manage consumer and program funds responsibly.Anticipated Work ScheduleMonday-Friday 8:30am-4:30pmOn-Call RotationOne full week every four weeks (subject to variation based on staffing levels)Job RequirementsAssociate degree preferred in a medical field with three years’ experience in related setting.Current, valid state driver’s license and acceptable driving record according to Hope Network policies.Our strength lies in diversity – empowering us to meet the distinct needs of over 34,000 individuals we're honored to serve each year.
