This position is established to support case managers by implementing authorized changes, services and contacts for PASSPORT and Assisted Living Waiver individuals.
DetailsConducts phone calls with individuals following hospital/nursing home discharge to complete care transition activity.
Mails zone management tool to individuals following discharge with qualifying diagnosis.
Assists the case manager by providing individual/informal support information to assist in gaining access to any necessary services regardless of funding source which enables individuals to remain in their community living arrangement.
Accurately and timely completes changes to the amount, scope, and duration of waiver services at the direction of the case manager.
Documents emails or voice mails from service providers in case notes and make corresponding adjustments to the service plan.
Contacts service providers or individuals/informal support to monitor service delivery and resolve service provision problems.
Completes service suspends in collaboration with case manager.
Conducts timely home visits, Level of Care and Comprehensive assessments as needed, according to program specifications.
Accurately maintains all appropriate documentation in formats and time frames prescribed by the PAA and the Ohio Dept. of Aging.
Utilizes electronic tools and software to complete tasks. LSW's or RN's will have an interactive contact with, primarily, older adult clients by phone and in-person visits

RequirementsPosition RequirementsLicensed Practical Nurse holding an active license to practice in the State of Ohio
Ability to travel in a multi-county service area and access private homes and facilities, which may not be readily accessible.
Ability to carry job related items weighing up to 30 pounds into private homes and facilities which may not be readily accessible.
Ability to relate well with all individuals, community agencies, and Area Agency staff.
Strong verbal and written communication skills.
Knowledgeable in Microsoft Word and basic computer skills.
Successful completion of the Ohio Dept. of Aging certification required within 12 months of employment.
Ability to communicate professionally, respectfully, and effectively.
Ability to make sound decisions and demonstrate critical thinking skills.
Ability to prioritize, organize and maintain accuracy in a flexible job environment.
