Job Title: Licensed Practical Nurse – Long Term Care-167874 Location: Muncy, PA 17756 Pay: $35.00 per hour Schedule: 5 x 8-hour shifts (40 hours per week) · Day and evening shifts available (exact shift discussed during interview-flexibility required) Overview We are seeking an experienced LPN to support residents in a Long-Term Care (LTC) / Skilled Nursing Facility (SNF) setting. In this role, the LPN provides direct patient care, administers medications, monitors resident conditions, and supports the healthcare team in maintaining the overall health and well-being of residents. LPNs in long-term care play an essential role in supporting residents with chronic health conditions by delivering compassionate care, assisting with treatments, and ensuring residents receive the appropriate level of medical attention and support. Key Responsibilities Provide direct nursing care to residents in a long-term care / skilled nursing facility environment. Administer medications and treatments according to physician orders and facility protocols. Monitor and document resident health status, vital signs, and changes in condition. Assist with care planning and collaborate with RNs, physicians, and healthcare staff to support resident care. Support residents with daily health needs while promoting comfort, dignity, and safety. Maintain accurate medical documentation and comply with regulatory and facility guidelines. Respond to resident needs and assist in managing chronic health conditions commonly seen in LTC settings. Required Qualifications Licensed Practical Nurse (LPN) license required. BLS Certification required. Minimum 2+ years of LPN experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting required. CNA experience cannot be counted toward this requirement.
