This position assists patients and their families by providing care management, ensuring timely consultations and diagnostic appointments, providing educational information regarding diagnosis, treatment, community resources, referrals and any other assistance needed in order to ensure patients are receiving the maximum benefits from medical care. In collaboration with the patient's primary care provider, the Primary Care Coach will assist patients with basic health education regarding chronic diseases, finding ways to take their daily medications, increase their physical activity, set and meet goals to improve their health. Estimated salary range for this position is $50457.98 - $65595.37 / year depending on experience.QualificationsDegrees:High School Diploma, Certificate of Attendance, Certificate of Completion, GED or equivalent training or experience required.Licenses & CertificationsBasic Life Support.Additional QualificationsMedical Assistant and Pediatric Advanced Life Support Certifications preferred.Must be a professional, detail-oriented team player with effective interpersonal communication skills.Confidentiality a must.Proficient in computer applications and typing skills.Experience in Electronic Medical Record systems preferred.Bilingual preferred.Completion of BHMG Health Coach educational program and passing test, within 6 months of employment.Minimum Required Experience: 3 yearsEOE, including disability/vets
