Start your amazing journey with us now!"Job Description: Case Management CoordinatorRequirements:Independent licensure with Supervisory Endorsement (LPCC-S, LISW-S, IMFT-S)1-2 years leadership experience, preferredExperience working as a case manager, preferredResponsibilities:Work and Clinical Supervision of Team of Case ManagersOverseeing Case Manager job responsibilities (Crisis Intervention, Mental Health Support and Education, Referrals to Community Resources and Agencies, Home Visits, Professional Development and Training, Prevention and Education Groups)Monitoring High Risk Tracking SheetsMonitor documentation of Case ManagersMonitor Referral and Active Student SpreadsheetsProviding weekly supervision sessions with each Case Manager individuallyDocumenting supervision sessions (Keeping supervision log and addressing issues below)Staffing casesSelf Care/Wellness of Case ManagerLegal/Ethical IssuesAdministrative Issues or School Building Issues/Communication with School PersonnelDocumentation/Outcomes/Communication SpreadsheetsProviding monthly team meetings with Case ManagersProviding Catalyst updates, new community resources, etcEstablishing meeting agendas and documenting meeting minutesSupporting and providing coaching for Case Managers between formal meetingsInformal Check Ins and Availability for Crisis ConsultationTraining and education of Case Managers based on strengths and areas for growthRelationship building with District administrators (building admin, counselors, teachers)Training Case Managers about relationship buildingOrganizational skillsCommunication SkillsKnowledge of community resources, referral sources, etcCrisis Intervention SkillsAttending weekly supervision meetings with Clinical Services Consultant to review Case Management programming, supports needed and clinical consultationManaging and updating any changes needed to Case Management Job ResponsibilitiesWill carry a caseload of therapy clients as well
