Physical Therapist
Description:The Physical Therapist (PT) provides evidence-based physical therapy services to patients across the lifespan, focusing on restoring functions, improving mobility, relieving pain, and preventing disability. The PT works with primary care providers, specialists, and interdisciplinary care teams to deliver coordinated, patient-centered care in an outpatient community health setting.
