CAREGIVER/PERSONAL CARE ASSISTANT
Description The caregiver/personal care assistant is responsible for assisting our clients with the support necessary to maintain and/or gain as much independence and community engagement as possible while ensuring they are healthy, safe and well. Reporting Relationship Reports to Supervisor and/or Executive Director. Responsibilities/Activities:
1. Accompanies clients on age-appropriate community activities.
2. Assists clients in meeting community living related goals and outcomes.
3. Assists with the activities of daily living and personal care including but not limited to: - bathing - shaving - ambulation - mouth care - dressing - exercise - hair care - feeding - toileting - nail care - positioning - medication reminding - skin care - transferring - vital signs and Blood Pressure
4. Assists with personal care such as bathing, dressing, ambulation, and transfers
5. Accompanies clients to the grocery store, medical appointments and pharmacy visits
6. Assists clients with active and passive mobility as defined by the plan of care, and documenting observed changes to their family, clinical staff and clinical providers.
7. Provides self-administration oversight medication with appropriate training and consent.
8. Ensures clients? safety and security by supervising the home and community environment.
9. Observes clients and their environments and report unsafe conditions to Supervisor.
10. Observes clients and their environments and reports behavior, physical and/or cognitive changes and/or changes in living arrangements to Supervisor.
11. Provides companionship including social interactions, conversations, emotional reassurance and encouragement of activities that stimulate the mind and body.
12. Provides respite care for caregivers in accordance with plans of care.
13. Assists with essential functions, which may include homework, community activities, age-appropriate household activities, etc..
14. Assists clients with following a written, special diet plan and reinforcement of diet maintenance, which is provided under the direction of their clinical provider and as identified on the care plan.
15. Escorts clients to medical facilities and appointments and outings as specified in the plan of care.
16. Participates on the Care Team by providing input and making suggestions at plan of care meetings.
17. Ensures service is delivered in accordance with all relevant regulations, policies, procedures, and protocols.
18. Monitors supplies and resources.
19. Evaluates the care plan during the course of services and makes recommendations for adjustments as necessary.
20. Closely follows the written plan of care.
21. Carries out other duties as assigned by the Supervisor.
22. Completes and maintains records of daily activities, observations, and direct hours of service and submits in a timely manner.
23. Attends orientation, in-service training sessions and staff meetings.
24. Develops and maintains constructive and cooperative working relationships with other key stakeholder. 25. Makes decisions and solves problems.
26. Communicates with clients, their family members/representatives, supervisors and co-workers consistently, respectfully and professionally.
27. Observes, receives and obtains information from relevant sources.
28. Performs other duties as required
Require Knowledge, Skills and Abilities
1. High school diploma or GED
2. Must be 18 years or older
3. First aid and CPR certification
4. Negative TB skin test, assessment or chest x-ray
5. Licensed driver with insured and reliable automobile
6. Adhere to HIPPA and maintain client confidentiality
7. Ability to read, write, speak and understand English fluently and communicate effectively
8. Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
9. Knowledge of community and home management skills.
10. Knowledge of principles and processes for providing community living related services and preventing crisis situations.
11. Knowledge of the information and techniques needed to diagnose and treat injuries including emergency first aid and CPR certification.
12. Knowledge of clerical procedures such as maintaining records, completing forms and documentation.
13. The ability to competently assist clients with their activities of daily living.
14. The ability to participate in activities with the client that assists him or her in working toward community and independent living related goals and outcomes.
15. The ability to understand written and oral instructions. Physical and Mental Demands:
1. Good physical and mental health.
2. Physical ability to stand, walk, life, use hands and fingers, reach, stoop, kneel, crouch, speak, hear and see.
3. Mental fortitude and stability to handle stress.
4. Physical, mental and legal ability to drive a vehicle.
Qualifications/Education
1. At least 18 years or older; and
2. Has a high school diploma or GED; or
3. U.S. citizen, work authorization or valid work permit.
4. Current driver?s license
5. Proper Vehicle Insurance Coverage.
6. Consistent, reliable transportation. Training/Experience:
1. Requires related experience.
2. CPR and First Aid Certification.
3. Requires Agency and job-related training.
