Field Nurse Practitioner (Full-Time)
The Nurse Practitioner provides comprehensive primary and preventive care in diverse settings such as in-home visits, community clinics, and nursing facilities, aiming to improve member outcomes while ensuring cost-effective care.Essential duties include performing evaluations, managing chronic and acute conditions, developing care plans, coordinating post-discharge care, and participating in community outreach.They drive up to 120 miles daily, often working independently, collaborating with physicians, and sometimes performing procedures or prescribing medications.Qualifications includeat least 1 year of NP experience,active national certification, a valid state license, DEA registration, and BLS certification.Strong communication, problem-solving, and adaptability skills are required, with preferred experience in home or community health and serving underserved populations.Travel, documentation, and teamwork are integral to the role.
