Case Coordinator

Job Overview We are seeking a dedicated and highly organized Case Coordinator to join our home care team. The ideal candidate will possess a strong background in senior home care settings, with experience in managing complex client cases, responsible to assign clients and their scheduled services. The position helps with reassessing and updating care needs and must able to know how to handle senior care. As a Case Coordinator, you will play a vital role in facilitating seamless patient care transitions, coordinating services and scheduling, and ensuring compliance with company regulations. Your expertise in management will contribute significantly to high-quality care and communication in our senior home care environment. Responsibilities Conduct comprehensive assessments to determine care needs and develop individualized care plans. Coordinate with family, social workers, behavioral health specialists, and other healthcare providers to ensure integrated client care. Support client, family and employee and follow-up care within hospital admission and re-admission. Manage smooth transitions with scheduling and assigning employee to the case based on the client's needs and employee's experiences and availabilities. Oversee documentation, care plans, and utilization review processes in accordance with HIPAA regulations. Monitor patient progress through evaluation and ongoing assessment to adjust care plans as necessary. Professional delicacy/etiquette phone answering, texting, and emailing Recruit direct care staff and marketing company service Perform office duties and on call rotation. Able to perform client care if in a pinch. Qualifications Some college degree preferred Proven experience working within senior home heath care or related environments. Strong knowledge of medical terminology, physiology, Dementia, Alzheimer and Parkinson diseases. Background in senior in-home care, mental health counseling, or social work preferred. Familiarity with care in-take and re-evaluation, and case management practices. Experience working with medical records systems while maintaining strict adherence to HIPAA guidelines. Ability to coordinate multidisciplinary teams effectively within the environments. Knowledge of managed care processes.
