Job Description
We are looking to hire a CUA Case Manager with experience in:Foster Care / Adoption / IHPS / SCOHJob Responsibilities:•Conduct intakes, provide assessment to determine family, child and youth needs•Provide case management services according to the DHS Community Umbrella Agency Practice Guidelines, Bethanna Policy, Procedures and practice standards as well as applicable regulations, contract standards and laws•Assess child, youth, home and family safety during service contacts•Report suspicions of abuse or neglect and any Unusual Incidents immediately to a Bethanna supervisor and appropriate legal authorities such as DHS Hotline and Childline•Provide home, school and community visits to assess need and assist in accessing resources•Develop a comprehensive Single Case Plan (i.e. service plan)•Collaborate with each family to develop the Single Case Plan ensuring their engagement and participation in developing goals•Facilitate referrals and access to additional resources as needed•Team collaboration with other components of youth and family services and serving systems•Create and maintain client files and complete all required documentation within expected time frames•Provide on-call services through a rotational basis•Provide written or an oral reports to department of human services and family court as needed•Participate in required training, Participate in supervision•Must demonstrate genuine empathy and concern for individuals as indicated in our mission statement and guiding principles _________________________________________________________________________________________Qualifications:•A bachelor's level in Human Services, BSW preferred•Two years case management experience involving youth•Knowledge of child and family service systems•Valid Driver's License (issued by state in which employee lives) and safe driving record and proof of insurance coverage•Experience with computers beneficial•Knowledge of social work values and/or child welfare services.Company DescriptionBethanna is a non-profit Christian organization that provides the highest quality system of care for children and families to ensure safety, restore emotional wellness, and build family stability.
