Job Title: Licensed Practical Nurse (LPN) – LTC / Rehab Department: Nursing Reports To: Director of Nursing / Charge Nurse Job Summary The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care and rehabilitation setting under the supervision of an RN or physician. The LPN supports resident recovery, promotes safety, and ensures high-quality, compassionate care. Essential Duties & Responsibilities Provide direct nursing care to residents in accordance with care plans Administer medications and treatments as ordered Monitor and document resident condition, vital signs, and changes in status Assist with admissions, discharges, and transfers Collaborate with RNs, physicians, therapists, and interdisciplinary team members Perform wound care, catheter care, and other skilled nursing tasks Assist residents with activities of daily living (ADLs) as needed Educate residents and families on care plans and treatments Maintain accurate and timely medical records Follow infection control, safety, and regulatory guidelines Respond to emergencies and provide basic life support as needed Qualifications Current LPN license in good standing Graduate of an accredited practical nursing program Experience in LTC or rehab preferred (new grads may be considered) Knowledge of nursing practices, medication administration, and documentation Ability to work independently and as part of a team Strong communication and organizational skills
