Implement the music therapy interventions in accordance with the interdisciplinary plan of care.Direct other care team members including, when appropriate, hospice aides and family members in the use of music to achieve individualized care plan goals.Evaluate the effectiveness of music therapy services with the patient. Document all services provided to hospice patients and families in the clinical record. Terminate therapy services when appropriate.Support the business development activities of the organization.Provide in-services to hospitals, nursing homes, and other facilities as requested and appropriate.Educate the hospice staff about the role of music therapy and the use of music therapy intervention with hospice patients and families.Actively collaborate and/or assist the Volunteer Department with their Indirect and Direct Music Volunteer program(s).Certifications, Licenses, and Other Special RequirementsMust be Board Certified as a Music Therapist by the Certification Board for Music Therapists (CBMT).Additional Hospice and Palliative Care Music Therapy (HPMT) Certificate preferred.Must have a valid driver's license.About Lifesong Hospice and Palliative Care:Lifesong co-workers connect with our clients in the comfort of their home by personalizing hospice care plans tailored to their preferences and needs and helping them transition to be as comfortable as possible.
