Every year, we proudly serve and connect more than 20,000 New Yorkers to healthcare systems. Our programs equip immigrant populations with the knowledge to help them navigate the complexities of living in a new city and transition into post-secondary education and/or employment opportunities. At the same time, we also educate community members on their immigrant rights, healthy living, nutrition and health access, and create space for civic engagement.DescriptionRaisingHealth is seeking a Coordinator of Social Care. Under the direction of the Manager of Community Health, the Coordinator of Social Care helps implement and administer effective outreach programs or events. The ideal candidate is a high-performing and motivated individual who has a passion for helping people and is comfortable interacting with the community, providing public presentations, and is highly organized with the ability to present information clearly and concisely, both written and verbal.Responsibilities:• Collaborate with the Manager of Community Health to identify, create, and execute community engagement opportunities to actualize health care programming initiatives aligned to RaisingHealth's mission.• Oversee RaisingHealth's case management case load in collaboration with Community Health Partners and ensure quality of care for community members across all initiatives• Coordinate and lead outreach efforts to increase awareness of various health coverage access and social service initiatives, and enroll community members into RaisingHealth programs• Oversee internal and external referrals for cases to various services. Independently navigate complex cases to connect individuals to care• Represent RaisingHealth at community events, cultural festivals, health fairs, and in other public settings, facilitating activities and interacting with event participants by sharing information related to RaisingHealth programs and opportunities.• Outreach to agencies, organizations, partners, and stakeholders to conduct presentations, both in-person and utilizing online platforms.Qualifications:• Two (2) years of experience with case management is required. Experience in healthcare enrollment and access, public engagement, cultural competency work, planning, and/or conducting educational or recreational programs is a plus.• Community Health Worker certification is a huge plus.• Experience with navigating Medicaid, Medicare, or other related health coverage and access programs is a plus.• Education: High school diploma or GED equivalent required. Bachelor's degree in a related field preferred.• Ability to empathize, advocate, and represent the needs of the community served by RaisingHealth.• Previous success in establishing relationships with individuals and organizations of influence, including partner agencies and volunteers.• Language - Bilingual language skills required. AMPHS has several CHP roles that require one of the following: o Spanish/English skills o Chinese (Mandarin) / English skills• Computer literacy required. Excellent command of Google Suite, especially Docs, Sheets, and Slides. Prior experience with case management platforms such as PracticeFusion, Salesforce, or UniteUs is a plus.*This is not a remote role. An in-office presence in our Brooklyn offices is required.Salary
