Job Description Vivo HealthStaff is searching for a Care Manager/Care Coach in San Francisco.Details:Full-time 40 hoursInclude remote work, Health Center or Home Care visitsDUTIES / RESPONSIBILITIES:Work closely with participants, caregivers, providers to perform day-to-day activities with participants in the community, at healthcare provider's offices and in participant's homes.Serve as patient advocate and program liaison.Quickly develop relationships inside and outside of the organizationExpected to be a role model for healthy behavior/lifestyle choices and be able to operate with minimal supervision.QUALIFICATIONS:Graduate of an accredited college with nursing, social work or gerontology degree.Experience working with older adultsExperience in advocating for the health and well-being of older adults; empatheticFamiliarity with social service, health and community agencies in San FranciscoKnowledge and understanding of HIPAA regulationsExperience with motivational interviewing and active listening skillsExperience in facilitating positive behavioral changes in participants/clientsBENEFITS:- Medical, Dental, and Vision coverage- Retirement Savings Plan- Holidays (10 per year), vacation time, sick leave, and long-term disability insuranceSince 2016, Vivo HealthStaff has been providing both temporary and permanent opportunities to clinicians across the United States.
