Position OverviewAs an Advanced Practitioner at CapitalCare Family Practice Clifton Park, you will deliver full-spectrum family medicine services in a collaborative, team-based environment. You will practice to the full extent of your license with strong physician support, integrated clinical services, and scheduling designed to promote work–life balance and long-term professional satisfaction. ResponsibilitiesConduct comprehensive patient evaluations and manage acute and chronic conditionsOrder and interpret diagnostic studies, prescribe medications, and coordinate referrals as neededProvide preventive care, health maintenance, and patient educationCollaborate closely with physicians, nurses, and support staff to ensure continuity of careMaintain accurate, timely clinical documentation in accordance with organizational standardsPractice HighlightsCare across the lifespan– serving pediatric, adult, and geriatric populationsStructured five or four-day schedule available (8:00 AM – 4:30 PM)with a consistent daily workflow and protected midday administrative/lunch timeTelemedicine visits and secure patient portal, enhancing access and continuity of careEstablished, team-based environmentwith multiple physicians and Advanced Practitioners supporting a strong patient panelConvenient Clifton Park location, serving a growing suburban community in the Capital RegionCompensation: Starting at $125,000 for full-time CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.
