As a Licensed Practical Nurse, you will play a vital role in providing exceptional, person-centered care to our residents and patients.About the Role:As a Licensed Practical Nurse (LPN) at Aviata at West Palm Beach, you will: Provide direct patient care to residents and patients, assisting with activities of daily living under the direction of a professional nurse. Assist in the implementation of individualized treatment plans for each assigned resident. Supervise CNAs and participate in their evaluation. Oversee Falls Program and Pressure Sore Program. Provide nursing services and conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment.Responsibilities: Assist with administering medications and treatments as directed by the resident's treatment plan. Monitor and report changes in resident condition to the Nurse Supervisor and; identify potential safety hazards and take steps to mitigate them. Participate in quality assurance activities and quality improvement initiatives. Maintain accurate and detailed records of resident care and treatment. Collaborate with other healthcare professionals to deliver comprehensive care.Requirements: A current, unencumbered, active state license to practice as an LPN. Experience in Skilled Nursing/Rehabilitation facilities preferred.
