Licensed Practical Nurse (Union)

Ditmas Park Nursing & Rehab - The Licensed Practical Nurse (LPN), is responsible for providing basic medical attention to Patients and assessing their wellbeing as part of a larger medical team. Their duties include checking vital signs, recording a Patient's medical history and assisting Patients with hygiene.Duties and Responsibilities:Administer medications and nursing procedures to Patients as prescribed.Obtain and record Patient temperatures, blood pressures, pulse rates, and other vital signs using standard instruments and procedures.Observe, monitor, and record Patient symptoms and conditions to obtain and provide information for diagnosis and treatment; identify responses to medication or changes in health status, and reports status to a registered nurse, physician, or other licensed health care provider.Prepare Patient for or assists with examinations and treatments.Maintain charts and other records of Patient conditions and/or administered treatment.Educate and counsel Patient regarding their health, medications, and treatment goals and procedures, as appropriate.Monitor supply inventories, and completes requisition forms to ensure adequate supply levels.As necessary, observe and supervise the activities of unlicensed direct care staff, including the administration of medication, to ensure compliance with applicable requirements and report observations to higher-level staff.Education / CertificationCurrently registered in New York State or has possession of a valid permit to practice as a Licensed Practical Nurse.Demonstrates knowledge of the nursing processes.Experience:Previous Long term Experience in LPN role is preferable.Good investigative skills.Ability to write, educate and work independently.Job Type: Full timeBenefits:401(k)Dental insuranceHealth insurancePaid time offVision insuranceSchedule:8 hour shiftMonday to Friday and every other weekendWork Location:One location
