Job Description Description:Innovations Community Mental Health Center is an outpatient facility where we provide case management services, group therapy (PHP and IOP), individual therapy, family therapy, and medication management for adults, adolescents, and children who suffer from mild to severe mental illness and chronic mental illnesses including chemical dependency.At Innovations CMHC we provide a holistic and community-based approach to behavioral healthcare. One that encourages you to provide quality care service and positively impact individuals for the better.Responsibilities and Duties:Provides rehabilitative services through:Medically necessary supportive activitiesProvides crisis intervention, case management and psychosocial rehabilitation to adults and youths and their familiesProvides services in accordance with evidence-based practicesDevelops individualized recovery plans in collaboration with the consumer to assist in meeting the goals and objectivesImplement recovery plans and monitor progressParticipates in clinical supervision sessionsEducate family members regarding existing community services, and act as an advocate in connecting family members to these servicesServices are provided in the office and in the community. Please note that some QMHP positions are primarily in the community.Manages an assigned caseloadAssessment of problems in the client's life domains posing barriers to recoveryAssisting and linking clients to community resources to meet basic needsAdvocating and helping the client navigate the system to support access to needed resources.Monitoring compliance with the treatment planEducating the client regarding rehabilitative activities of daily living * Guides the client to reach goals, objectives and treatment outcomes.Collaborates with family members for the direct benefit of the client.Collaborates with referral sources and community agencies assisting the client.Attentive to the skills necessary to do assigned tasks efficiently and effectively.Stays knowledgeable of necessity standards pertaining to services delivered.Establish open lines of communication for individuals with mental or emotional issuesOffer assistance to individuals with substance abuse issuesCoordinate treatment planImplement and review treatment plansCollaborate with additional treatment providers and placement agenciesEnsure all documentation is completed in a timely mannerMust be able to work independently and efficiently within our EMR (Electronic Medical Record)Completes all notes and documentation in a timely, ethical, and high quality manner per Innovation CMHC's policies and procedures, following The Joint Commission and Medicare guidelines/standardsEngages in timely efficient patient case conferences in conjunction with physician, staff, patients and familiesReports any discrepancies related to therapists to immediate supervisorQualifications and Skills Requirements:Bachelor's or Master's degree from an accredited college or university with a minimum number of hours that are equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education, or early childhood intervention: or a licensed registered nurse.Familiarity with community mental health resourcesAt least three years of related experience and/or training in the field of mental healthMust be certified in ANSA and CANS assessmentsPrior working experience as a case managerRoutine Case ManagementSkills Training and DevelopmentMedication Education / Patient and Family EducationPsychosocial RehabilitationKnowledge of counseling skills and techniquesKnowledge of mental health services and agenciesKnowledge of mental health diagnosisDemonstrates effective problem solving skillsTeamwork/CooperationExcellent written and verbal communications skillsAbility to build rapport with clientsStrong leadership qualitiesMust be able to provide at least 3 professional or educational referencesAble to participate in First Aid, CPR, Verbal and Physical De-escalation Techniques TrainingValid Driver's LicenseMinimum insurance bodily injury liability coverageCompetenciesAbility to problem-solve and utilize good judgmentAbility to self-start and be a quick learnerAbility to prioritize; strong organizational skillsAbility to work in a fast-paced, high volume environmentAbility to be flexible and creative to meet the needs of consumersAbility to make home visits and transport consumersExcellent engagement skillsExcellent time management skillsExcellent oral and written communication skillsStrong knowledge of community resourcesExcellent computer skillsAbility to meet volume and documentation standardsAt Innovations Community Mental Health Center we care about the health and safety of our employees.
