Community Health Worker
The Community Health Worker (CHW) will fully integrate the multidisciplinary care team of Meharry Community Wellness Center (MCWC). He/she will develop a close relationship with patients, coach them and facilitate access to core and support services. He/she will identify, track, re-engage and monitor clients lost to care. The CHW will assist clients in dealing with competing priorities, adherence with medication, clinic visits, and specialty referrals, linkages with community-based organizations, wellness education, harm reduction, and navigation through the healthcare delivery system. The CHW will meet with clients in the clinic, at their homes, and different access points in the community.Essential Functions (Duties and Responsibilities):Adherence counseling, harm reduction, health promotion, and patient navigationCollect all required data elements and provide timely reportsOutreach services to persons living with HIV, through direct personal contact and collaborative community partnershipsLinkages with Meharry Community Wellness Center, community-based organizations and specialty servicesAttend all mandatory meetingsPerform other related duties as assigned by Supervisor or Executive Director.Knowledge, Skills and Abilities: Basic knowledge of HIV/AIDSBe a team player and well-organizedSkills in engaging people living with HIVAbility and willingness to use both knowledge and personal experience in facilitating linkage of individuals and groups to essential HIV health related servicesCommunication skills and proven behaviors consistent with maintenance of health and wellness.Ability to establish trust based relationships with people in an appropriate and ethical mannerKnowledge and ability to access community resourcesAbility to attend mandatory staff meetingsBasic computer skills and ability to collect data elements and generate reports in a timely mannerMotivated to serve people with HIV and get trained as assigned by Executive Director or supervisorEducation and Experience: High school diploma with commensurate experience; associate or bachelor degree welcomeFamiliarity with people living with HIV is mandatoryFamiliarity with Nashville/Davidson and/or surrounding CountiesFamiliarity with Ryan White programs and Meharry Community Wellness Center is highly desirableMust possess valid driver's licenseWillingness to attend all required trainingsReliability, punctuality, and disciplineEnvironmental Conditions and Physical Demands Usual office environment.Manual dexterityVisual and auditory acuityAble to sit, stand or walk for extended periods of timeAble to travel in Nashville/Davidson and surrounding Counties
