Rather than focusing on archaic outdated design, we strive to consistently question the status-quo and create new and more innovative ways to help aging adults and adults with disabilities maintain their quality of life./ppWith over 23 programs, we offer multiple ways to aid seniors maintain their health, well-being, independence and participation in the community, fulfilling our mission./ph3Primary Responsibility/h3pProvide care management services in the Acute Care Hospital including completion of intake services and care coordination for clients in San Francisco/ph3Essential Job Functions/h3ulliCompletes assessment and care plans as indicated by supervisor/liliEnsures program and staff compliance with all regulatory requirements./liliEnsures quality care and services to all clients./liliEngages in crisis intervention to support client needs as needed/liliResponsible for mentoring new staff during on boarding process/liliMaintains Quality Assurance mechanisms to measure and maintain high standards of care and programming, including regular chart reviews and peer review./liliComplies with all mandated reporting to support local and state requirements/liliDevelops and maintains collaborative working relationships and cooperative arrangements with team, supervisor, community individuals, groups and organizations/liliTakes advantage of opportunities to educate the public and the community regarding program./liliResponsible for ensuring that authorized and purchased goods/services are used as a last resort and that all community resources and referrals are made to appropriate waiver programs./liliActively participates in and encourages actions that promote good public relations with clients, their families and the community./liliResponsible for expanding personal knowledge, skill and attitudes specific to care management, family systems, geriatrics and gerontology, addiction and other relevant management or health care issues./liliProvides caseload coverage when Care Management staff are out of office or unavailable/liliAccepts and performs other related duties and responsibilities as required./li/ulh3Education/h3pMSW or Masters in related clinical field (ex: MFT), current license (LCSW, LMFT) preferred OR RN./ph3Background and Experience/h3ulliFive years experience working directly with disabled/older adults or related social/ medical experience required with at least one year demonstrated leadership skills./liliDemonstrated ability to supervise intensive care management staff who work with adults with disabilities at risk of hospitalization, which present with complex medical conditions, mental health diagnoses, substance abuse, as well as physical rehabilitation needs./liliDemonstrates understanding of behavioral health services and behavioral health interventions./liliProven previous training of staff as it relates to the populations served./liliDemonstrates understanding of the health and psychosocial needs of this population./liliDisplays knowledge of community resources available for this population./liliDemonstrates ability and willingness to take on program development projects./liliProven ability and willingness to provide leadership amongst peers and mentor junior staff./li/ulh3Compensation/h3ulliRange: $ 95,170.00 - $ 113,742.00 /annual/li/ulpThis amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors./ppBeware of Hiring Scams/pulliInstitute on Aging will never request payment or sensitive personal information such as Social Security numbers during the hiring process./liliAll official communication will come from a verified IOA email address./liliIf you receive any suspicious communication or requests, report them to talentacquisition@ioaging.org./liliAll legitimate job openings can be found on the Institute on Aging Careers Page./li/ulpInstitute on Aging is an Equal Opportunity Employer.
